In order to have your child play Football, all of the following must be completed and agreed with.
Complete the online registration form, parent and player info.
Complete and sign the Boys & Girls Club Youth Membership Application  and 
Complete and sign the Concussion Information Acceptance Form 

Make sure you bring both these forms "completed & signed" to our 1st Football Meeting as they are required in order to complete your Childs registration.
